All HIM baccalaureate and associate degree programs must submit this required report by this date to avoid being placed on Administrative Probationary Accreditation.

The Annual Program Assessment Report (APAR) is the online resource exclusively for CAHIIM Accredited Programs to document yearly programmatic outcomes information required for Maintenance of Continuing Accreditation.

This site is designed for CAHIIM internal use only and not available for general access. Accredited programs may continue to access previous APAR years at the system Login.

System Reminders

CAHIIM Quality Review Letter Link: The Quality Review Letter document is available from the Activity Menu when you login. Please click on "Reports" and then "View Historical Reports". In order to access your most recent review, you must select the year 07-08 and click on Quality Review Letter. NOTE: It is important that you DISABLE the Pop-up Blocker functionality in your browser tool bar.

Passwords: If you do not know your system password, please access the Retrieve Password link and follow the instructions.

Section I: Profile

Question: Is the complete HIM program offered as Distance Learning?

Please verify that the default selection is accurately reflected in the CAHIIM Accredited Program Directory. You should contact CAHIIM Staff at benjamin.reed@cahiim.org if your program's distance learning search feature is not accurate.

Section II:Program Metrics

All numbered fields must be populated. Leave the number as "0" if there is no count to report. The system will not accept negative numbers.

Attrition: Students should be counted who leave the HIM program during the 08-09 reporting year. If they come back in a succeeding year, they are just counted as enrolled for that applicable time frame or reporting year. Please use the "Comments" section for any needed explanations on trends and causes.

Previous Year data of 07-08 or 06-07 for all sections are not editable.

Section III: Placement and Satisfaction Rate

Programs should track cohorts of students for Graduate Placement. However the Graduate Satisfaction Rate is captured for responses "received" during the Academic Year that reflects 10/1/2008 - 9/30/2009.

Section IV: Employer Satisfaction

The information for the current year should be responses "received" during the Academic Year that reflects 10/1/2008 - 9/30/2009. This is not tracking cohorts of students that graduated during this timeframe.

Section V: Certification Outcomes

The information in this section is provided from a Cumulative Report (10/1/2008 - 9/30/2009) to all continuing programs from processes of the AHIMA Commission on Certification for Health Informatics and Information Management (CCHIIM). Please contact lisa.chernikoff@ahima.org if you have any questions or concerns on this report prior to reporting your results.

Section VI: Program Evaluation Plan

Current Goals: At minimum, the asterisked program goals are required goals that needs to have at least one target outcome.

Section VII: Substantive Changes (Administrative requirements for Maintaining Accreditation)

This section requires that programs document examples of the following categories of developments that occurred since the previous APAR:

Adverse changes in the program's institutional accreditation status;
Program inactivity or closure;
Change(s) in Program Director;
Distance education;
Satellite or branch campus affecting the HIM program.

Section VIII: Curriculum

Information reported in the previous year has been transferred over. This information should be updated and include any explanatory statements in the "Comments" section.
